I recall from reading about the Desert Shield deployment that the Time Phased Deployment List/Force Packages for the CENTCOM AOR were in the midst of a major rewrite when Saddam invaded Kuwait, which caused no end of hassle. (see p 43) So I think it might be a collection of plans (for each region/scenario) that evolved over time, and it might have been computerized rather than paper documents, which could be problematic!
