I don't know the model but a discussion on reddit concluded the ammo to be the following.

Okay, several Russian sites state that to be "An Old German Gun", however I cannot find a page with it actually on.

However, I have found 2 gunsmiths with similar guns.

Its a german high price huntig rifle/ shotgun.

Vierling = four barrels

Drilling = three barrels

In good old days in Germany there were legal restrictions on how many weapons you could own. Vierling or drilling is nice legal way to avoid those restrictions. After all you have shotgun, small- and large caliber rifle in same gun.

Those restrictions are long gone, but drillings are still weapon of choice (in Finland at least)to some hunters, who have nostalgic ideas about wild boar hunting in East Prussia (and have extra cash- those guns are quite expensive).
